摘要:
目的:比较前外侧和内侧入路经皮接骨板治疗胫骨远端骨折的临床疗效。方法:对2009年1月至2013年12月收治并完整随访的43例闭合性胫骨远端骨折患者的资料进行回顾性分析。根据切口入路不同分为前外侧组20例和内侧组23例,均采用经皮接骨板固定。术前两组一般资料比较差异无统计学意义(P>0.05),有可比性。比较两组手术时间、骨折愈合时间、术后完全负重行走时间、术后1年患肢功能和并发症情况。结果:43例患者经1.0～2.1年(平均1.4年)随访。比较两组患者手术时间、骨折愈合时间、术后负重行走时间、术后1年患肢功能,差异无统计学意义(P>0.05)。两组并发症比较,前外侧组明显优于内侧组,差异有统计学意义(P<0.05)。结论:前外侧和内侧入路经皮接骨板治疗胫骨远端骨折均能取得良好的临床疗效,前外侧入路的软组织并发症更少,当内侧软组织条件不佳时,前外侧入路是良好的选择。
Abstract:
Objective: To compare the clinical effectiveness of anterolateral and medial approach percutaneous plate in treatment of distal tibial fracture.Methods: From January 2009 to December 2013, 43 patients with close distal tibial fracture were treated and retrospectively analyzed, 20 cases in anterolateral group and 23 cases in medial group according to different approach.All patients were fixed by percutaneous plates.There was no significant difference between the two groups in the general data(P>0.05).The operative time, bone union, time to complete weight bearing, limb function and complications at 1 year postoperative were compared between the two groups.Results: The patients were followed up for 1.0～2.1 years(average 1.4 years).There was no significant difference in operative time, bone union, time to complete weight bearing, limb function at 1 year postoperative(P>0.05).The anterolateral group was better than medial group in complications with significant difference(P<0.05).Conclusion: Anterolateral and medial approach percutaneous plate were excellent for treating distal tibial fracture.Soft tissue complications of anterolateral approach were less,which was a good choice especially for bad soft tissue condition in medial leg.
