摘要:
目的:探讨经皮球囊扩张椎体后凸成形术(PKP)治疗老年新鲜与陈旧性骨质疏松椎体压缩骨折(OVCFs)的临床疗效。方法:选取骨质疏松性椎体压缩骨折患者98例,108个手术椎体,平均70.5岁。新鲜骨折组64例68椎:男8例,女56例; 年龄61~86岁,平均 69.5岁。陈旧性骨折组34例40椎:男8例,女26例; 年龄61~90岁,平均72.6岁。随访时间 6 ~12 个月,通过视觉模拟评分(VAS 评分),Oswestry 伤残指数(ODI),椎体前中柱的高度,脊柱后凸角度进行临床结果的评定。结果:两组治疗前后及术后12个月 VAS 评分、ODI 指数、伤椎前中柱椎体高度及后凸Cobb 角均较术前明显改善(P<0.05); 陈旧性骨折组治疗后伤椎前中柱椎体高度及后凸角较新鲜骨折组改善小。结论:椎体后凸成形术是治疗老年新鲜及陈旧性 OVCFs的一种有效的手段,治疗后疼痛症状可明显缓解,但陈旧性骨折较新鲜骨折来说术后椎体高度及后凸角恢复较小,活动功能改善明显较少。
Abstract:
Objective: To compare the clinical effect of percutaneous kypoplasty(PKP)for fresh and old osteoporotic vertebral compressive fractures(OVCFs). Methods: The study involved 98 patients with OVCFs treated by PKP from Dec 2010 to Aug 2011.There were 64 patients with fresh fractures(group A), 34 patients with old fractures(group B). Group A included 8 males and 56 females with an average age of 69.5 years(ranging from 61 to 86 years). Group B included 8 males and 26 females with an average age of 72.6 years(ranging from 61 to 90 years). Clinical outcome of all the patients was evaluated after operation through visual analog scale(VAS), Oswestry disability index(ODI), the height of anterior and middle column of the vertebral and the kyphotic angle. Results: The VAS score and the ODI were improved significantly after PKP in both groups(P<0.05). The post operative height of anterior and middle column vertebra and the kyphotic angle in group B were less improved than group A. In the follow-up period, 4 new fractures(2 adjacent)appeared in the group A, while 3 new fractures(1 adjacent)in the group B. Conclusions: PKP is effective for patients with fresh or old painful OVCFs in functions of pain relief and functional recovery. The risk of new fracture and long-term efficacy need further research.
