摘要:
目的:探讨股骨转子间骨折患者实施内固定术后发生髋内翻的危险因素,为临床诊治提供参考。方法:选择2011年1月-2014年4月股骨转子间骨折患者108例,所有患者均实施内固定术治疗,回顾性分析患者基线资料、骨折分型、内固定方式等潜在影响因素对髋内翻影响,确定危险因素。结果:单因素分析结果显示年龄≥60岁(χ2=3.214,P=0.038)、BMI>25 kg/m2(χ2=4.035,P=0.021)、骨密度T值≤-2.5 SD(χ2=6.224,P=0.007)、骨折分型(χ2=7.219,P=0.002)、小转子未复位(χ2=4.316,P=0.019)、术后负重时间≤2月(χ2=6.387,P=0.005)是髋内翻相关因素; 多因素分析结果提示,骨密度(95%CI=2.124~9.586,P=0.001)、骨折分型(95%CI=2.323~10.334,P=0.000)、术后负重时间(95%CI=2.541~11.541,P=0.000)是影响髋内翻的独立危险因素。结论:骨密度、骨折分型、术后负重时间是影响髋内翻的独立危险因素。
Abstract:
Objective: To explore the risk factors of varus occurred in intertrochanteric fractures treated by internal fixation, providing a reference for clinical diagnosis and treatment.Methods: 108 cases of intertrochanteric fracture from January 2011 to April 2014 were treated with internal fixation. Retrospectivily analysised the potential impact factors(such as baseline data, fracture type, fixation methods)on hip varus, then identify the risk factors. Results: Univariate analysis showed that age ≥60 years(χ2=3.214, P=0.038), BMI>25 kg / m2(χ2=4.035,P=0.021), bone mineral density T-score ≤-2.5 SD(χ2 =6.224,P=0.007), fracture type(χ2=7.219,P=0.002), reseted lesser trochanter(χ2=4.316,P=0.019)and postoperative weight loading time≤2 months(χ2=6.387,P= 0.005)were related factors of hip varus. Multivariate analysis results suggestted that bone mineral density(95% CI =2.124~9.586, P=0.001), fracture type(95% CI=2.323~10.334,P=0.000)and postoperative weight loading time(95% CI=2.541~11.541,P=0.000)were independent risk factors of hip varus. Conclusion:BMD, fracture type and postoperative weight loading time were independent risk factors for hip varus.
