目的:研究非特异性下腰痛患者椎旁软组织肌张力和痛阈值的特点,明确理筋手法在非特异性下腰痛治疗中的最佳干预期,提高临床疗效,完善治疗规范化。方法:90例非特异性下腰痛患者,按诊断标准分为急性组(25例)、亚急性组(29例)、慢性组(36例),治疗前后测量肌张力和痛阈值,分析理筋手法最佳干预期。结果:1)手法治疗后3组硬度值均较治疗前有所降低,急性组手法治疗后P=0.569,差异无统计学意义; 亚急性组手法治疗后,P=0.000,差异有统计学意义; 慢性组手法治疗后P=0.148,差异无统计学意义; 2)痛阈值手法治疗后,急性组手法治疗后痛阈值较治疗前降低,P=0.001,差异有统计学意义; 亚急性组手法治疗后较治疗前升高,P=0.000,差异有统计学意义; 慢性组手法治疗后较治疗前有所升高,P=0.248,差异无统计学意义。结论:理筋手法对亚急性非特异性下腰痛具有较高的敏感性和特异性。
Abstract:
Objective:To study the characteristics of muscle tension and pain threshold of paravertebral soft tissue in patients with nonspecific low back pain(NLBP),and to explore the best intervention time of manipulation in the treatment of NLBP,and to further improve the clinical efficacy and standardization of treatment.Methods:90 patients with NLBP were divided into acute group(25 cases),sub-acute group(29 cases),and chronic group(36 cases)according to the diagnostic criteria.The muscle tension and pain threshold were measured before and after treatment,and the the best intervention time of manipulation were analyzed.Results:1)The hardness values of muscle tension were decreased in all three groups after manipulation treatment.There was no significantly difference in the acute group and chronic group(P=0.569,P=0.148),but there was significantly difference in the sub-acute group(P=0.000).2)The pain threshold of paravertebral soft tissue in acute group was significantly decreased after treatment(P=0.001),but were significantly increased after treatment in sub-acute group(P=0.000).There was no difference in chronic group after treatment(P=0.248).Conclusion:The manipulation is sensitivity and specificity on sub acute group of NLBP,which can improve the muscle spasm,reduce soft tissue tension,and improve local pain threshold.
