摘要:
目的:探讨补肾除湿方联合关节镜下自体软骨移植治疗早中期膝骨关节炎的疗效与安全性。方法:收集2023年11月至2024年12月共45例膝骨关节炎患者的临床资料,用随机数字表法分为观察组(25例)和对照组(20例),分析术前、术后2周及4周骨关节炎疼痛指数(WOMAC)评分、Lequesne指数评分、膝关节活动度及炎症因子变化。结果:两组患者术前WOMAC评分及Lequesne指数评分比较差异无统计学意义(P>0.05)。两组患者术后2周及4周测量WOMAC评分、Lequesne指数评分、膝关节活动度均优于术前,差异有统计学意义(P<0.05); 两组患者术前血清指标白细胞介素6(IL-6)、基质金属蛋白酶13(MMP-13)、基质金属蛋白酶3(MMP-3)比较差异无统计学意义(P>0.05)。术后2周及4周测量血清指标IL-6、MMP-13、MMP-3水平均优于术前,差异有统计学意义(P<0.05)。结论:补肾除湿方联合关节镜下自体骨软骨移植术能明显改善早中期膝骨关节炎患者疼痛症状,改善患肢关节功能。
Abstract:
Objective:To explore the efficacy and safety of the Bushen Chushi formula combined with arthroscopic autologous osteochondral transplantation(AOT)in the treatment of early- to middle-stage knee osteoarthritis(KOA).Methods:The clinical data of 45 patients with knee osteoarthritis,collected from November 2023 to December 2024,were analyzed.Patients were divided into an observation group(n=25)and a control group(n=20)using a random number table.The WOMAC osteoarthritis pain index scores,Lequesne index scores,knee joint range of motion,and levels of inflammatory factors were analyzed before surgery,2 weeks after surgery,and 4 weeks after surgery.Results:There was no statistically significant difference in the WOMAC and Lequesne index scores between the two groups before surgery(P>0.05).The WOMAC osteoarthritis pain index scores,Lequesne index scores,and knee joint range of motion of both groups were better than those before surgery 2 weeks and 4 weeks after surgery(P<0.05).There was no statistically significant difference in the serum indicators of interleukin 6(IL-6),matrix metalloproteinase 13(MMP-13),and matrix metalloproteinase 3(MMP-3)between the two groups before surgery(P>0.05).The serum indicators of IL-6,MMP-13,and MMP-3 measured 2 weeks and 4 weeks after surgery were better than those before surgery(P<0.05).Conclusion:The combination of the Bushen Chushi formula and the arthroscopic autologous bone and cartilage transplantation surgery can significantly alleviate the pain symptoms of patients with early- to mid-stage knee osteoarthritis and improve the joint function of the affected limb.
