摘要:
膝骨关节炎是全球范围内导致中老年人群残疾的主要原因之一,随着老龄化进程加速,已成为一项突出的公共卫生问题。单纯西医或中医干预仍面临疾病修饰性药物缺失、单一靶点治疗局限性及难治性中枢敏化等诸多瓶颈。系统梳理了中西医结合防治膝骨关节炎的体系化优势:该模式通过多靶协同有效破解难治性痛觉敏化; 通过重塑宏观生物力学与微观免疫代谢双重稳态,延缓软骨退变进程; 并在全程管理中实现西药减毒增效,深度赋能加速康复外科。未来应强化基础治疗,构建“医-患-家”协同网络,并建立多维高质量的循证评价体系,为提升膝骨关节炎全生命周期管理效能提供中国方案。
Abstract:
Knee osteoarthritis(KOA)is a leading cause of disability among middle-aged and elderly populations globally,emerging as a prominent public health issue with the accelerating aging process.Interventions relying solely on Western or traditional Chinese medicine continue to face numerous bottlenecks,including the absence of disease-modifying osteoarthritis drugs(DMOADs),the limitations of single-target therapies,and intractable central sensitization.This paper systematically reviews the systemic advantages of integrating Chinese and Western medicine in KOA prevention and treatment.This integrative model effectively resolves intractable pain sensitization through multi-target synergy; delays cartilage degeneration by remodeling the dual homeostasis of macroscopic biomechanics and microscopic immunometabolism; and reduces toxicity while enhancing the efficacy of Western medications throughout whole-course management,deeply empowering enhanced recovery after surgery(ERAS)protocols.Future efforts should focus on strengthening foundational treatments,constructing a “doctor-patient-family” collaborative network,and establishing a multidimensional,high-quality,evidence-based evaluation system,thereby providing a Chinese solution for enhancing the full life-cycle management of KOA.
