摘要:
目的:探究单侧双通道脊柱内镜术中37 ℃恒温冲洗液对患者体温及局部炎症介质的影响。方法:选择2024年6月至2025年8月行单侧双通道脊柱内镜手术患者60例,随机分为对照组和观察组。两组手术均由同一名经验丰富的主任医师进行,观察组采用37 ℃恒温生理盐水冲洗,对照组采用25 ℃室温生理盐水冲洗。通过监测术中温度、术后12 h、1周及3周的VAS评分及引流液中炎症因子TNF-α、IL-6水平评价疗效。结果:手术60 min时,对照组患者核心体温显著下降至(35.5±0.3)℃,低体温发生率为86.67%(26/30); 观察组体温为(36.2±0.4)℃,低体温发生率为36.67%(11/30),组间差异均有统计学意义(P<0.05)。术后1周及3周,观察组VAS评分均显著低于对照组(P<0.05),术后12 h两组VAS评分差异无统计学意义(P>0.05)。在术后3 h及术后6 h,观察组引流液TNF-α及IL-6水平均显著低于对照组,差异有统计学意义(P<0.05)。结论:单侧双通道脊柱内镜术中使用37 ℃恒温冲洗液可有效维持患者围术期核心体温稳定,降低低体温发生率,并减轻局部炎症反应,有助于缓解术后疼痛。
Abstract:
Objective:To explore the effect of 37 ℃ constant-temperature irrigation fluid on core body temperature and local inflammatory mediators in patients undergoing unilateral biportal endoscopic surgery(UBE).Methods:Sixty patients who underwent UBE surgery from June 2024 to August 2025 were randomly divided into control group and observation group.Both groups were operated on by the same experienced chief physician.The observation group was irrigated with normal saline maintained at 37 ℃,while the control group was irrigated with normal saline at room temperature(25 ℃).The efficacy was evaluated by monitoring the intraoperative core body temperature,VAS scores at 12 h,1 week,and 3 weeks postoperatively,and the levels of inflammatory factors TNF-α and IL-6 in the drainage fluid.Results:At 60 min of the operation,the core body temperature of the patients in the control group significantly decreased to(35.5±0.3)℃,with a hypothermia incidence rate of 86.67%(26/30); the body temperature of the observation group was(36.2±0.4)℃,and the hypothermia incidence rate was 36.67%(11/30).The differences between the two groups were statistically significant(P<0.05).One week and three weeks after the operation,the VAS scores of the observation group were significantly lower than those of the control group(P<0.05).There was no statistical difference in VAS scores between the two groups 12 h after the operation(P>0.05).At 3 h and 6 h after the operation,the levels of TNF-α and IL-6 in the drainage fluid of the observation group were significantly lower than those of the control group(P<0.05).Conclus-ion:The use of 37 ℃ constant temperature irrigation fluid during UBE can effectively maintain the stability of core body temperature during the perioperative period,reduce the incidence of hypothermia,and alleviate local inflammatory responses,which is helpful to relieve postoperative pain.
