摘要:
针对腰椎间盘突出症诊疗中存在的影像与症状脱节、治疗针对性不足等问题,总结谢兴文主任医师的临证经验。通过将腰椎间盘突出症分为单纯腰痛型、下肢放射痛型、下肢麻木型及复合型四类,基于“精准分型-多法协同”的治疗原则,单纯型以三步三位九法手法松解通络,放射型配合针刺辨经止痛,麻木型首创腰痛外洗方化瘀荣筋,复合型多法联用。该体系体现了病症结合、筋骨并重的治疗思路,临床应用显示能有效改善患者症状,可为腰椎间盘突出症的中西医结合诊疗提供规范化的实践范式。
Abstract:
In view of the problems existing in the diagnosis and treatment of lumbar disc herniation(LDH),such as the disconnection between images and symptoms and the lack of targeted treatment,this paper summarizes the clinical experience of Chief Physician Xie Xingwen.LDH is divided into four types:simple low back pain type,lower limb radiation pain type,lower limb numbness type,compound type.Based on the treatment principle of “precise classification-multi-method coordination”,the simple type uses “three steps,three positions and nine methods” to relax the collaterals; radiation therapy combined with acupuncture is used to differentiate meridians and alleviate pain; numbness is the first external washing prescription for lumbago to remove blood stasis and strengthen tendons; and a compound,multi-method integrative approach is adopted.The system embodies the treatment principle of integrating symptoms with bones and muscles.The clinical application shows that it can effectively improve the symptoms of patients and provide a standardized practice paradigm for the diagnosis and treatment of LDH using integrated traditional Chinese and Western medicine.
