摘要:
目的:观察超声引导下手法复位治疗儿童桡骨小头成角移位骨折的临床疗效。方法:2023年7月至2025年1月,收集采用此方法治疗的儿童桡骨小头成角移位骨折患者30例临床资料,随访记录骨折愈合时间及并发症情况,末次随访时评估肘关节功能。结果:14例Metaizeau评分为2分的骨折患儿复位后变成1分,即基本复位; 16例Metaizeau评分为3分的患者中有14例骨折复位后变成1分,2例由3分转变到2分(轻度移位); 所有病例骨折均愈合,均未出现相关并发症; 1例患儿因不配合固定,导致骨折移位,后改为手术治疗。复位后12周末次随访时肘关节功能评分优25例,良4例,手术患儿功能评分为一般,总体优良率为96.6%。结论:肌骨超声引导下手法复位儿童桡骨小头成角移位骨折,骨折愈合和肘关节功能恢复好,且安全性高。
Abstract:
Objective:To evaluate the clinical efficacy of ultrasound-guided closed reduction in the treatment of radial neck “button-cap” fractures.Methods:From July 2023 to January 2025,30 cases of pediatric radial neck “cap” fractures treated with this method were collected.The fracture healing time and complications were recorded during follow-up,and elbow joint function was evaluated at the last follow-up.Results:Among the 14 cases with a Metaizeau score of 2,the fractures of the children were reduced to grade 1,indicating essentially complete reduction.Among the 16 cases with a Metaizeau score of 3,14 cases improved to grade 1 after fracture reduction,and 2 cases changed from grade 3 to grade 2 with mild displacement.All cases of fractures healed without any related complications.In one case,the child's fracture displacement,due to lack of cooperation with fixation was later managed with surgical treatment.At the 12-month follow-up,the elbow joint function score was excellent in 25 cases and good in 4 cases.The functional outcome for the surgically treated children was favorable,with an overall excellent and good rate of 96.6%.Conclusion:Under the guidance of muscle ultrasound,closed reduction of radial neck “mislabeled” fractures achieves good fracture healing and elbow joint function recovery,with high safety.
