摘要:
目的:探究双向牵引闭合复位机器人导航精准置钉在跟骨骨折微创治疗Sanders Ⅱ/Ⅲ型跟骨骨折的临床疗效及安全性。方法:选取新鲜闭合跟骨骨折86例,采取双向牵引结合“撬、顶、夹”闭合复位与机器人辅助螺钉内固定术进行治疗,记录患者围术期指标,并对机器人辅助经皮置钉在跟骨骨折中的疗效和安全性进行评价。结果:所有患者受伤至手术时间为1~5 d,中位数为2 d,均在机器人辅助下顺利完成手术,一次性螺钉置入成功率为100%,术中无血管、神经损伤等严重并发症。所有患者手术切口均甲级愈合。手术时间为(44.91±11.36)min。3个月复查随访时摄跟骨轴侧位X线片,提示骨折均愈合。患者均获随访,随访时间为12~18个月,中位数为14个月。术后未出现切口感染、钉道感染、皮缘坏死及深部组织感染等情况。跟骨Böhler角、Gissane角、宽度、高度、长度恢复良好。术后5个月Maryland评分为(84.29±6.04)分,优良率为94.19%。结论:双向牵引结合“撬、顶、夹”等闭合复位技术,联合机器人辅助螺钉内固定,治疗跟骨骨折疗效显著,无需术前等待消肿,未来有可能转化为日间手术,尤其在一次性螺钉置入成功率、微创等方面优势明显,具有一定的临床应用价值。
Abstract:
Objective:To explore the clinical efficacy and safety of dual-directional traction closed reduction technology and robot-assisted screw placement in Sanders Ⅱ/Ⅲ calcaneal fractures.Methods:Eighty-six cases of fresh closed calcaneal fractures were selected.All patients were treated with dual directional traction combined with closed reduction techniques and robot-assisted screw fixation.The perioperative indicators were recorded,and the clinical efficacy and safety were evaluated.Results:The time from injury to operation was 15 d,with a median of 2 d.The success rate of one-time screw placement and the success rate of operation were both 100%.There were no serious complications,such as vascular or nerve injuries during the operation.All patients' surgical incisions healed with Grade A healing.The operation time was(44.91±11.36)min.The X-ray images of the calcaneal axis and lateral position were taken during the 3-month follow-up,indicating that the fractures had healed.All patients were followed up for 1218 months,with a median of 14 months.No incision infection,nail tract infection,skin edge necrosis,or deep tissue infection occurred in any of the patients.The Böhler angle,Gissane angle,width,height,and length of the calcaneus recovered well after surgery.At 5 months postoperatively,the Maryland score was(84.29±6.04)points,and the excellent and good rate of Maryland score was 94.19%.Conclusion:The curative effect of dual-directional traction calcaneal closed reduction technology combined with robot-assisted screw internal fixation in the treatment of calcaneal fractures is significant.This operation does not need to wait for detumescence before proceeding,and may be converted to daytime operation in the future.It has obvious advantages in the success rate of one-time screw implantation,minimally invasive surgery,and other aspects,and has certain clinical application value.
