摘要:
目的:探讨苍柏丹对骨科大手术后下肢深静脉血栓形成及炎症的影响。方法:将50只大鼠随机分为空白组、模型组、苍柏丹低/高剂量组和阳性药物组,每组各10只,空白组正常喂养。除空白组外各组进行骨科大手术后大鼠下肢深静脉血栓模型造模,造模5 d后模型组和空白组大鼠开始生理盐水灌胃2次(2.5 mL/次),生理盐水0.3 mL皮下注射1次; 苍柏丹低剂量组大鼠临床1倍剂量中药(6.84 g/kg)灌胃2次,生理盐水0.3 mL皮下注射1次; 苍柏丹高剂量组大鼠临床2倍剂量中药(13.68 g/kg)灌胃2次,生理盐水0.3 mL皮下注射1次; 阳性药物组大鼠生理盐水灌胃2次(2.5 mL/次),低分子肝素钙注射液0.3 mL皮下注射1次。连续用药5 d,取大鼠大隐静脉1~2 cm石蜡包埋切片,苏木精-伊红(HE)染色法、免疫组化和Western Blot法检测各组血管内皮细胞内人核因子κB抑制蛋白α(NF-κB Inhibitor α,IκBα)和p65 蛋白表达; 腹主动脉采血用酶联免疫吸附测定法(ELISA)检测血浆白细胞介素(IL)-6、IL-8、高迁移率族蛋白B1(HMGB1)浓度。结果:大隐静脉HE染色可见苍柏丹和阳性药物对血管内膜发挥保护作用,高剂量组保护效果更好。ELISA检测各组大鼠血清IL-6、IL-8、HMGB1的表达水平,差异有统计学意义(P<0.05); 空白组<阳性药物组,苍柏丹高剂量组<苍柏丹低剂量组<模型组,苍柏丹高剂量组与阳性药物组比较差异无统计学意义(P>0.05)。Western Blot法检测IκBα和p65的蛋白表达:空白组<阳性药物组,苍柏丹高剂量组<苍柏丹低剂量组<模型组,差异有统计学意义(P<0.05); 苍柏丹高剂量组与阳性药物组比较差异无统计学意义(P>0.05)。免疫组化法检测IκBα和p65的蛋白表达:空白组<阳性药物组,苍柏丹高剂量组<苍柏丹低剂量组<模型组,差异有统计学意义(P<0.05); 苍柏丹高剂量组与阳性药物组比较差异无统计学意义(P>0.05)。结论:苍柏丹发挥其清热祛湿、活血祛瘀的功效,通过抑制NF-κB通路的异常活化,调控NF-κB p65、IκBα的mRNA和蛋白表达,进而有效抑制炎症反应,从而治疗下肢深静脉血栓形成。
Abstract:
Objective:To evaluate the effects of Cangbai Dan on deep vein thrombosis(DVT)formation and inflammation after major orthopedic surgery.Methods:Fifty rats were randomly assigned to five groups(n=10 each):blank,model,low-dose Cangbai Dan,high-dose Cangbai Dan,and positive drug group.Except for the blank group,all rats underwent DVT modeling.From day 5 post-surgery,rats received intragastric administration of Cangbai Dan(6.84 g/kg or 13.68 g/kg,twice daily),saline(2.5 mL,twice daily),or saline combined with low-molecular-weight heparin calcium(0.3 mL,subcutaneously,once daily)for five consecutive days.Histopathological changes in the great saphenous vein were evaluated by hematoxylin-eosin(HE)staining.Expression of IκBα and p65 proteins was assessed by immunohistochemistry and Western Blot.Serum IL-6,IL-8,and HMGB1 levels were measured by ELISA.Results:HE staining showed that Cangbai Dan and the positive drug protected the vascular endothelium,with superior effects in the high-dose group.ELISA demonstrated that IL-6,IL-8,and HMGB1 levels were lowest in the blank group and highest in the model group,with dose-dependent reductions in the Cangbai Dan groups.No significant difference was observed between the high-dose Cangbai Dan group and the positive drug group(P>0.05).Western Blot and immunohistochemistry showed consistent trends in IκBα and p65 expression.Conclusion:Cangbai Dan prevents postoperative lower limb DVT by inhibiting NF-κB pathway activation,downregulating NF-κB p65 and IκBα expression,and attenuating inflammatory responses.These findings support its therapeutic potential in the prevention of thrombosis after orthopedic surgery.
