摘要:
目的:探讨富血小板血浆(PRP)联合系统功能训练对前交叉韧带重建(ACLR)术后寒区高运动功能需求患者的康复效果。方法:选取2024年1-11月收治的70例长期在寒区工作生活的前交叉韧带断裂重建术后患者,依照治疗方案划分成两组。常规组(33例)进行系统功能训练,联合组(37例)在常规组基础上进行富血小板血浆治疗。比较两组患者治疗前后的疼痛程度(VAS评分)、运动功能(Lysholm量表评分、膝关节活动度(ROM)、国际膝关节评分委员会(IKDC)膝部主观评估量表评分)、生存质量(膝关节效果调查-生活活动量表(KOS-ADL)及前交叉韧带损伤后重返运动量表(ACL-RSI)评分)、等速肌力及不良反应发生情况。结果:经治疗后,两组患者VAS评分降低,差异有统计学意义(P<0.05),联合组VAS评分小于常规组,差异有统计学意义(P<0.05); Lysholm评分、IKDC评分、膝关节活动度、KOS-ADL评分、ACL-RSI评分、伸膝及屈膝的力矩峰值均升高,差异有统计学意义(P<0.05),且联合组相较于常规组均显著升高,差异有统计学意义(P<0.05)。联合组和常规组的不良反应总发生率分别为18.19%和12.12%,两组比较差异无统计学意义(P>0.05)。结论:对于前交叉韧带断裂重建术后的寒区高运动功能需求患者,富血小板血浆与系统功能训练的联合应用,能够在康复中期强化阶段明显减轻疼痛程度,加速运动功能的恢复,并显著提升其生存质量。
Abstract:
Objective:To explore the rehabilitation effects of platelet-rich plasma(PRP)combined with systemic functional training on patients with high physical activity level in cold regions after anterior cruciate ligament reconstruction(ACLR).Methods:From January to November 2024,we selected 70 patients who had undergone ACLR surgery and had been living and working in cold climates for extended periods.They were then divided into two groups based on their treatment plans.The routine group(n=33)adopted systematic functional training,while the joint group(n=37)adopted PRP in addition to the routine group.Two groups were compared in terms of pain(Visual Analogue Scale(VAS)),motor function(Lysholm Scale,Range of Motion(ROM),Tegner Knee Movement Score,and International Knee Documentation Committee(IKDC)Subjective Knee Evaluation Form),and quality of life(Knee Outcome Survey Activities of Daily Living Scale(KOS-ADL),Anterior Cruciate Ligament-Return to Sport after Injury(ACL-RSI)scores),as well as isokinetic muscle strength and the occurrence of adverse reactions before and after treatment.Results:Both groups had decreasd VAS scores after treatment(P<0.05),with the joint group having a lower VAS score than the routine group(P<0.05).Both groups showed an increase in IKDC score,Tegner score,ROM,KOS-ADL score,ACL-RSI score,and the peak moments of both knee extension and knee flexion(P<0.05),and the joint group showed a greater increase than routine group(P<0.05).The total incidence of adverse reactions in the joint group and the routine group was 18.19% and 12.12%,respectively,and there was no significant difference between the two groups(P>0.05).Conclusion:For patients in cold regions with high demands for motor function after ACLR surgery,the combined application of PRP and systematic functional training can effectively reduce pain levels during the mid-rehabilitation intensification phase,accelerate the recovery of motor function,and significantly improve their quality of life.
