摘要:
目的:研究浮针治疗膝骨关节炎(KOA)的临床疗效及生物力学评价。方法:选择2023年6月至2024年8月收治的80例膝骨关节炎患者,随机分为普通针刺组和浮针组,每组各40例。普通针刺组予以普通针刺治疗,浮针组予以浮针治疗,均治疗2周。比较两组患者治疗前和治疗2周后疼痛程度、膝关节功能、实验室指标(白细胞计数(WBC)、C反应蛋白(CRP)、红细胞沉降率(ESR))及步态分析,另比较两组患者临床疗效与安全性。结果:两组患者治疗2周后视觉模拟量表(VAS)评分和西安大略和麦克马斯特大学(WOMAC)骨关节炎指数中疼痛、僵硬、日常活动评分,以及白细胞计数、C反应蛋白、红细胞沉降率水平均低于治疗前,差异有统计学意义(P<0.05),但浮针组上述指标低于普通针刺组,差异有统计学意义(P<0.05); 两组患者治疗2周后步速、步频、步长均高于治疗前,差异有统计学意义(P<0.05),但浮针组上述指标高于普通针刺组,差异有统计学意义(P<0.05); 浮针组总有效率(94.87%)高于普通针刺组(76.92%),差异有统计学意义(P<0.05); 浮针组和普通针刺组均无明显不良反应。结论:浮针治疗膝骨关节炎的临床疗效显著,可明显减轻疼痛和炎症水平,改善膝关节功能和步行能力,且安全性较好,具有较高的临床应用价值。
Abstract:
Objective:To investigate the clinical efficacy and biomechanical evaluation of floating needle therapy for knee osteoarthritis(KOA).Methods:80 KOA patients admitted from June 2023 to August 2024 were selected,and they were randomly divided into two groups,with 40 patients in each group.The regular acupuncture group received regular acupuncture treatment,while the floating needle group received floating needle treatment,both treatment for 2 weeks.The pain level,knee joint function,laboratory indicators(white blood cell count(WBC),C-reactive protein(CRP),erythrocyte sedimentation rate(ESR))and gait analysis before and after 2 weeks of treatment between two groups were compared,and the clinical efficacy and safety of the two groups were compared.Results:After 2 weeks of treatment,the visual analogue scale(VAS)scores and pain,stiffness,daily activity scores on the Western Ontario and McMaster University(WOMAC)osteoarthritis index and WBC,CRP,ESR levels in both groups were lower than those before treatment(P<0.05),but the above indexes in the floating needle group were lower than those in the regular acupuncture group(P<0.05).After 2 weeks of treatment,the walking speed,step frequency and stride length of both groups were higher than those before treatment(P<0.05),but the above indicators in the floating needle group were higher than those in the regular acupuncture group(P<0.05).The total effective rate of the floating needle group(94.87%)was higher than that of the regular acupuncture group(76.92%)(P<0.05).There were no obvious adverse reactions in floating needle group and regular acupuncture group.Conclusion:The clinical efficacy of floating needle therapy for KOA is significant,which can effectively reduce pain and inflammation levels,improve knee joint function,improve walking ability,and it has good safety and high clinical application value.
