摘要:
目的:观察针刀联合体外冲击波治疗髌腱末端病的临床疗效及安全性。方法:回顾性分析2023年9月至2024年11月就诊的髌腱末端病患者54例,采用针刀联合体外冲击波治疗,7 d为1个疗程,共治疗4个疗程。记录并分析治疗前、首个疗程后及疗程结束后3周的疼痛视觉模拟量表(VAS)评分、改良Lysholm膝关节评分、维多利亚运动评估研究所髌腱肌腱病评分量表(VISA-P)评分及超声下检测髌腱厚度。结果:疗程结束3周后VAS评分为(2.08±1.33)分,低于治疗前的(7.26±1.10)分和首个疗程后的(3.26±1.46)分; 疗程结束3周后改良Lysholm评分为(88.20±6.74)分,高于治疗前的(61.52±7.51)分和首个疗程后的(82.69±7.23)分; 疗程结束3周后VISA-P评分为(84.50±3.86)分,高于治疗前的(62.30±3.86)分和首个疗程后的(75.02±5.95)分; 疗程结束3周后髌腱厚度为(5.64±0.74)mm,低于治疗前的(7.45±1.20)mm和首个疗程后的(7.80±1.14)mm; 差异均有统计学意义(P<0.001)。临床治疗总有效率为92.59%。结论:针刀联合体外冲击波治疗髌腱末端病具有较好的临床疗效,操作简单、安全可靠,可明显改善患者的膝关节功能,提高患者生活质量,值得临床推广应用。
Abstract:
Objective:To observe the efficacy of acupotomy combined with extracorporeal shock wave therapy in treating patellar tendon enthesopathy.Methods:A retrospective analysis was conducted on 54 patients diagnosed with patellar tendon enthesopathy from September 2023 to November 2024.Treatment involved acupotomy combined with extracorporeal shock wave therapy,administered in 4 courses(7 d per course).Pain levels(visual analogue scale,VAS),functional status(Victoria Institute of Sport Assessment for Patellar Tendinopathy,VISA-P),and patellar tendon thickness under ultrasound were recorded and analyzed before treatment,after the first course,and 3 weeks post-treatment.Results:3 weeks after treatment completion,VAS scores decreased to(2.08±1.33)points,compared to(7.26±1.10)points pre-treatment and(3.26±1.46)points after the first course.After 3 weeks of treatment,the improved Lysholm score was(88.20±6.74)points,which was higher than the pre-treatment score of(61.52±7.51)points and the first course score of(82.69±7.23)points.VISA-P scores improved to(84.50±3.86)points,versus(62.30±3.86)points pre-treatment and(75.02±5.95)points after the first course.Patellar tendon thickness reduced to(5.64±0.74)mm,compared to(7.45±1.20)mm pre-treatment and(7.80±1.14)mm after the first course.All differences were statistically significant(P<0.05).The total clinical effective rate was 92.59%.Conclusion:Acupotomy combined with extracorporeal shock wave therapy demonstrates notable clinical efficacy for patellar tendon enthesopathy,offering advantages such as simplicity,safety,and reliability.It effectively improves knee joint function and warrants clinical promotion.
