摘要:
目的:运用三维有限元仿真技术建立C5/6 颈椎间盘突出模型,探究颈椎定点旋转扳法对不同退变程度下椎间盘的应力-应变效应,揭示其治疗椎间盘源性颈椎病的生物力学机制。方法:构建不同退变程度下C5/6椎间盘左后外侧突出的三维有限元模型,经验证后模拟颈椎定点旋转扳法操作及力学加载,观察手法作用后纤维环突出区域位移与髓核压力变化。结果:手法作用后,不同退变模型椎间盘纤维环均呈顺时针旋前移动,中度退变模型位移值最大; 髓核最大应力随退变程度加重而上升,最小应力区域逐渐扩大,且应力集中区域与突出区域一致。结论:颈椎定点旋转扳法可改善椎间盘异常应力分布,对椎间盘源性颈椎病的治疗有潜在应用价值,不过临床应用需考量椎间盘退变程度,重度退变者手法宜谨慎。
Abstract:
Objective:To use three-dimensional finite element simulation technology to establish a model of C5/6 cervical intervertebral disc herniation,explore the stress-strain effect of the fixed-point rotational manipulation of the cervical spine on the intervertebral discs with different degrees of degeneration,and reveal the biomechanical mechanism of its treatment of discogenic cervical spondylosis.Methods:A three-dimensional finite element model of the left posterolateral protrusion of the C5/6 intervertebral disc under different degrees of degeneration was constructed.After verification,the operation of the fixed-point rotational manipulation of the cervical vertebrae and the mechanical loading were simulated,and the changes in the displacement of the protrusion area of the annulus fibrosus and the pressure of the nucleus pulposus after the manipulation were observed.Results:After the manipulation,the annulus fibrosus of the intervertebral discs in different degeneration models all moved in a clockwise pronation direction,and the displacement value of the moderately degenerated model was the largest.The maximum stress of the nucleus pulposus increased with the aggravation of the degree of degeneration,the area of the minimum stress gradually expanded,and the stress concentration area was consistent with the protrusion area.Conclusion:The fixed-point rotational manipulation of the cervical vertebrae can improve the abnormal stress distribution of the intervertebral disc,and has potential application value in the treatment of discogenic cervical spondylosis.However,the degree of intervertebral disc degeneration needs to be considered in clinical application,and the manipulation should be used with caution for patients with severe disc degeneration.
