摘要:
目的:对比低温等离子射频消融术(LTPA)与射频热凝术(RFTC)分别联合胶原酶治疗腰椎间盘突出症(LDH)的有效性和安全性。方法:选取2023年6月至2024年12月进行治疗的100例腰椎间盘突出症患者作为研究对象,按照随机数字表法将其分为A组(50例,低温等离子射频消融术联合胶原酶)和B组(射频热凝术联合胶原酶),在治疗6个月后对患者的临床疗效、疼痛介质(5-羟色胺(5-HT)、前列腺素E2(PGE2)、β-内啡肽(β-EP))、炎症因子(白细胞介素-1β(IL-1β)、IL-17和基质细胞衍生因子(SDF-1))、疼痛程度(视觉模拟量表(VAS)评分)、腰椎功能(日本骨科协会(JOA)评分、Oswestry功能障碍指数(ODI))及并发症情况进行对比分析。结果:经过治疗,两组患者的5-HT、PGE2、IL-1β、IL-17、SDF-1、VAS评分及ODI评分显著降低,β-EP及JOA评分显著升高。A组患者的临床疗效、5-HT、PGE2、IL-1β、IL-17、SDF-1、VAS评分及ODI评分较B组显著降低,差异有统计学意义(P<0.05),其β-EP及JOA评分较B组显著升高,差异有统计学意义(P<0.05),但两组患者的并发症差异无统计学意义(P>0.05)。结论:低温等离子射频消融术与射频热凝术联合胶原酶治疗腰椎间盘突出症,可以明显减缓患者的疼痛,改善其腰椎功能,且低温等离子射频消融术联合胶原酶的疗效优于射频热凝术联合胶原酶治疗。
Abstract:
Objective:To compare the efficacy and safety of low-temperature plasma assisted adenoidectomy(LTPA)and radiofrequency thermocoagulation(RFTC)combined with collagenase in treating lumbar disc herniation(LDH).Methods:From June 2023 to December 2024,100 patients with LDH were recruited as study subjects.They were randomly assigned into group A(50 cases,combination of LTPA with collagenase)and group B(combination of RFTC with collagenase)through random number table.After 6 months of treatment,clinical efficacy,pain mediators(5-hydroxytryptamine(5-HT),prostaglandin E2(PGE2),β-endorphin(β-EP)),inflammatory factors(interleukin-1β(IL-1β),IL-17,and stromal cell-derived factor(SDF-1)),pain severity(visual analogue scale(VAS)scores),lumbar spine function(Japanese Orthopaedic Association(JOA)scores,Oswestry disability index(ODI)scores,and complications were compared and analyzed.Results:After treatment,5-HT,PGE2,IL-1β,IL-17,SDF-1,VAS scores and ODI scores were significantly decreased in both groups,while β-EP and JOA scores were significantly increased.Group A had conspicuously reduced clinical efficacy,5-HT,PGE2,IL-1β,IL-17,SDF-1,VAS scores,and ODI scores than group B(P<0.05),and conspicuously increased β-EP and JOA scores than group B(P<0.05).However,there was no conspicuous difference in complications(P>0.05).Conclusion:The combination of LTPA or RFTC with collagenase can effectively alleviate pain and improve lumbar function in LDH,and the efficacy of combination of LTPA with collagenase is superior to combination of RFTC with collagenase.
