摘要:
目的:探讨骨搬运联合抗生素骨水泥技术治疗老年开放性胫骨骨折术后感染伴骨外露的临床疗效。方法:回顾性分析2018年3月至2023年8月收治的老年开放性胫骨骨折术后感染伴骨外露患者18例。男17例,女1例; 年龄为60~79岁; 采用骨搬运联合抗生素骨水泥技术治疗,记录创面愈合时间、骨断端会师时间、外固定时间和并发症发生情况。末次随访时采用Paley评分标准评价骨折愈合情况,采用Johner-Wruhs评分标准评价患肢功能。结果:患者随访14~25个月。创面愈合时间为49~94 d,骨折断端会师时间为57~125 d,外固定时间为281~512 d。末次随访时Paley评分结果:优6例,良11例,中1例。Johner-Wruhs评分结果:优4例,良9例,中5例。随访过程中患者无发热、创面流脓、软组织坏死及马蹄内翻足畸形等并发症。结论:骨搬运联合抗生素骨水泥技术治疗老年开放性胫骨骨折术后感染伴骨外露可有效控制感染,重建骨和软组织缺损,促进骨折和创面愈合,恢复肢体功能。
Abstract:
Objective:To investigate the efficacy of bone transport combined with antibiotic bone cement technology in the treatment of elderly patients with postoperative infection and bone exposure after open tibial fracture.Methods:A retrospective analysis was performed on 18 elderly patients with postoperative infection and bone exposure after open tibial fracture from March 2018 to August 2023.There were 17 males and 1 female,with age of 6079 years old.They were treated with bone transport combined with antibiotic bone cement technology.The wound healing time,fracture docking time,external fixation time and the occurrence of complications were recorded.At the last follow-up,the Paley score was used to evaluate the fracture healing,and the Johner-Wruhs score was used to evaluate function of the affected limb.Results:The patients were followed up for 14 to 25 months.The wound healing time was 4994 d.The fracture docking time was 57125 d.The external fixation time was 281512 d.At the last follow-up,the results were 6 excellent cases,11 good cases and 1 moderate case according to Paley score.The results were 4 excellent cases,9 good cases and 5 moderate cases according to Johner-Wruhs score.During the follow-up,there were no complications such as fever,wound discharge,soft tissue necrosis and talipes equinovarus deformity.Conclusion:For elderly patients with postoperative infection and bone exposure after open tibial fracture,bone transport combined with antibiotic bone cement technology can control infection,reconstruct bone and soft tissue defects,promote fracture and wound healing,and restore limb function.
