摘要:
目的:探讨骨皮质剥离术联合富血小板血浆(PRP)治疗骨搬移术后会师端骨不愈合的临床疗效。方法:选取2021年6月至2024年6月24例因骨搬移术后会师端骨不愈合手术治疗患者的临床资料,采用随机数字表法将患者分为对照组和观察组(各12例)。对照组患者采用开放断端清理联合髂骨取骨植骨治疗,观察组患者采用骨皮质剥离术联合富血小板血浆治疗。观察骨折愈合时间、手术时间、术中出血量、感染率、骨折愈合率并评估患者相关疗效。结果:24例患者均获得完整随访,随访时间为9~16个月,平均为(12.7±1.2)个月,24例患者均骨性愈合。观察组患者与对照组的手术时间、术中出血量、并发症发生率差异有统计学意义(P<0.05),观察组均优于对照组。对照组患者出现1例伤口浅表感染,4例患者遗留髂骨取骨区疼痛。结论:骨皮质剥离术联合富血小板血浆是治疗骨搬移术后会师端骨不愈合的一种有效的方法,具有手术时间短、出血少、并发症少等优势。
Abstract:
Objective:To investigate the clinical efficacy of cortical peeling combined with platelet-rich plasma in the treatment of bone nonunion at the junction after bone transport.Methods:The clinical data of 24 patients who underwent surgery for nonunion at the junction after bone transport from June 2021 to June 2024 were selected.The patients were divided into control group and observation group by random number table method,12 cases in each group.The control group was treated with open debridement combined with iliac bone grafting.The observation group was treated with cortical peeling combined with platelet-rich plasma.The fracture healing time,operation time,intraoperative blood loss,infection rate,fracture healing rate and related therapeutic effects were observed.Results:All 24 patients were followed up for 916 months,with an average of(12.7±1.2)months.All the 24 patients achieved bony union.There were significant differences in operation time,intraoperative blood loss and incidence of complications between the observation group and the control group(P<0.05),and the observation group was better than the control group.In the control group,1 patient had superficial wound infection,and 4 patients had pain in the iliac bone harvesting area.Conclusion:Cortical peeling combined with platelet-rich plasma is an effective method for the treatment of bone nonunion at the junction after bone transport,which has the advantages of shorter operation time,less bleeding,and fewer complications.
