摘要:
目的:观察藤黄健骨片对肾虚血瘀型膝骨关节炎(KOA)大鼠的软骨保护作用,探讨其作用机制。方法:采用去卵巢、激素和肾上腺素联合注射、木瓜蛋白酶和L-半胱氨酸关节腔内注射等复合因素法构建肾虚血瘀型膝骨关节炎大鼠模型,随机分组(n=8):假手术组、模型组、藤黄健骨片高剂量组(1.00 g/kg)、藤黄健骨片低剂量组(0.50 g/kg)、阳性组(双醋瑞因0.05 g/kg)。灌胃给药6周,于给药前后观察并记录各组大鼠的整体体征,采用改良奎森功能障碍指数(Lequesne MG评分)评价膝关节功能; 给药后ELISA法检测血清白细胞介素1β(IL-1β)、转化生长因子β(TGF-β)水平,番红O-固绿染色法观察关节软骨和软骨下骨组织病理变化并进行国际骨关节炎研究学会(OARSI)软骨退变评分,免疫组化法检测关节软骨中基质金属蛋白酶13(MMP-13)、Ⅱ型胶原(C Ⅱ)阳性表达。结果:模型大鼠表现出典型的肾虚血瘀证候,Lequesne MG评分均≥6分; 与模型组比较,藤黄健骨片高、低剂量组Lequesne MG评分显著降低,差异有统计学意义(P<0.01),藤黄健骨片高剂量组Lequesne MG评分显著低于低剂量组,差异有统计学意义(P<0.01)。藤黄健骨片高、低剂量组在关节软骨表层平整度、软骨细胞排列、软骨下骨组织结构破坏等病变方面均有所改善,OARSI软骨退变分级明显优于模型组,差异有统计学意义(P<0.05)。与模型组比较,藤黄健骨片高、低剂量组血清IL-1β水平明显降低,TGF-β水平明显增高,差异有统计学意义(P<0.01); 藤黄健骨片高剂量组血清IL-1β水平显著低于低剂量组,差异有统计学意义(P<0.05)。与模型组比较,藤黄健骨片高、低剂量组明显抑制关节软骨MMP-13阳性表达,差异有统计学意义(P<0.01),显著增强C Ⅱ阳性表达,差异有统计学意义(P<0.01)。结论:藤黄健骨片可明显改善肾虚血瘀型膝骨关节炎大鼠的整体体征和膝关节症状、保护关节软骨,与纠正IL-1β和TGF-β等细胞因子的表达失衡、抑制软骨MMP-13表达并增强C Ⅱ表达、减少软骨细胞和细胞外基质的降解等功能有关。
Abstract:
Objective:To investigate the cartilage-protective effects of Tenghuangjiangu tablet(TT)in rats with knee osteoarthritis characterized by syndrome of kidney deficiency and blood stasis(KOA-KDBS),and to understand its mechanism of action.Methods:A rat model of KOA-KDBS was created using a combination of methods including ovariectomy,co-injection of hormones and epinephrine,and intra-articular injections of papain and L-cysteine.The rats were randomly assigned to different groups(n=8):sham-operated group,model group,high-dose TT group(1.00 g/kg),low-dose TT group(0.50 g/kg),and positive control group(diacerein 0.05 g/kg).The overall signs of the rats in each group was monitored and recorded,knee joint function was assessed using the modified Lequesne MG score before and after the treatment.After 6 weeks of treatment,serum levels of IL-1β and TGF-β were measured using the ELISA method,the articular cartilage and subchondral bone tissues were examined using the safranin O-fast green staining technique,pathological changes in the articular cartilage and subchondral bone were evaluated using the OARSI score,and the expression of MMP-13 and collagen type Ⅱ(C Ⅱ)in the articular cartilage was analyzed through immunohistochemistry.Results:The model rats exhibited typical symptoms of KOA-KDBS,with Lequesne MG scores all at or above 6.When compared to the model group,the Lequesne MG scores for the TT high and low dosage groups were significantly lower(P<0.01),with the high dosage group showing a significantly greater reduction than the low dosage group(P<0.01).Both dosage groups demonstrated improvements in the flatness of the articular cartilage surface,arrangement of chondrocytes,and the integrity of subchondral bone tissue structure,with OARSI scores significantly better than that of the model group(P<0.05).Additionally,the serum IL-1β levels in both dosage groups of TT were significantly reduced compared to the model group,while TGF-β levels increased significantly(P<0.01); the high dosage group had a significantly lower IL-1β level than the low dosage group(P<0.05).Both dosage groups also significantly inhibited the positive expression of MMP-13 in articular cartilage(P<0.01)and significantly enhanced the positive expression of C Ⅱ(P<0.01)compared to the model group.Conclusion:TT effectively improve overall signs and knee joint symptoms while protecting the articular cartilage in KOA-KDBS rats.This effect is associated with the correction of cytokine expression imbalances,specifically IL-1β and TGF-β,the inhibition of cartilage MMP-13 expression,the enhancement of C Ⅱ expression,and a reduction in chondrocyte and extracellular matrix degradation.
