摘要:
目的:观察慢性非特异性下腰痛患者臀部医用红外热成像表现与表面肌电信号特点,探讨医用红外热成像在臀肌活化状态评估中的意义。方法:选取慢性非特异性下腰痛患者30例,提取患者红外热成像图计算双侧臀肌体表温度,用表面肌电信号评估患者双侧臀肌活化状态,用广义线性Logistic回归模型分析臀肌的医用红外热成像表现和表面肌电信号特点相关性。结果:患者健侧平均温度(33.81±0.35)℃显著高于患侧平均温度(32.56±0.27)℃(t=15.488,P<0.001)。患者健侧竖脊肌和臀大肌的反应时间、肌电振幅峰值均显著高于患侧(反应时间t=2.855,12.950; 肌电振幅峰值t=2.583,2.629,均P<0.05)。患者双侧臀肌体表温度与其收缩反应时间差负线性相关(竖脊肌OR=3.381,3.351,3.401,3.585; 臀大肌OR=3.779,3.238,3.685,3.238,均P<0.05),与其静力性收缩时表面肌电振幅峰值正线性相关(竖脊肌OR=0.623,0.618; 臀大肌OR=0.534,0.712,均P<0.001)。结论:慢性非特异性下腰痛患者臀肌医用红外热成像表现及表面肌电信号特点具有相关性,医用红外热成像相对低温可视化该侧臀肌肌力及活化状态与对侧相比较弱,高温则相反,应用医用红外热成像可为臀部肌肉活化状态的评估提供有效信息。
Abstract:
Objective:To observe the medical infrared thermography(MIT)performance and surface electromyographic signal(sEMG)characteristics of the gluteal region in patients with chronic non-specific low back pain(CNLBP),and to explore the significance of MIT in the assessment of gluteal muscle activation status.Methods:30 patients with CNLBP were selected,infrared thermograms of the patients were extracted to calculate the body surface temperature values of the gluteal muscles bilaterally,sEMG was used to assess the activation status of the patients' gluteal muscles bilaterally,and the generalized linear Logistic regression model was used to analyze the correlation between the MIT manifestations of the gluteal muscles and the sEMG characteristics.Results:The mean temperature value of the patient's healthy side(33.81±0.35)℃ was significantly higher than the mean temperature value of the affected side(32.56±0.27)℃(t=15.488,P<0.001).The reaction time and peak EMG amplitude of the erector spinae and gluteus maximus muscles on the healthy side of the patients were significantly higher than those on the affected side(reaction time t=2.855,12.950,peak EMG amplitude t=2.583,2.629,all P<0.05).The patient's bilateral gluteal muscle body surface temperature values showed a negative linear correlation with their contraction reaction time difference(erector spinae OR=3.381,3.351,3.401,3.585,gluteus maximus OR=3.779,3.238,3.685,3.238,all P<0.05),and a positive linear correlation with their peak surface electromyographic amplitude during static contractions(erector spinae OR=0.623,0.618,gluteus maximus OR=0.534,0.712,all P<0.001).Conclusion:There is a correlation between the MIT performance and sEMG characteristics of the gluteal muscle in patients with CNLBP.The MIT relative low temperature visualizes the weaker gluteal muscle strength and activation status on that side compared with the contralateral side,while high temperature is the opposite,and the application of the MIT can provide effective information for the assessment of the activation status of the gluteal muscle.
