摘要:
目的:探讨行气利湿方联合金黄膏治疗对湿热痹阻型膝骨关节炎(KOA)的影响。方法:选取2023年1月至2024年6月接受治疗的膝骨关节炎患者105例,按随机数字表法分为联合用药组(行气利湿方+金黄膏)、中药外用组(金黄膏)和西药组(口服塞来昔布胶囊+迈之灵片),并完成4周的治疗。在治疗2周和治疗4周时,比较两组患者的肿胀程度(目测评估、浮髌试验分级、患膝周径)、疼痛程度(VAS评分)、功能评定(HSS评分)、滑膜炎程度(积液厚度测量、滑膜厚度分级)、WORMS评分等。结果:三组患者在治疗2周时和治疗4周时肿胀程度、疼痛程度和功能评分均优于治疗前,差异有统计学意义(P<0.01),联合用药组在治疗后的疼痛程度和功能评分优于中药外用组和西药组,差异有统计学意义(P<0.01); 联合用药组在治疗后的滑膜炎程度评分和WORMS评分优于中药外用组和西药组,差异有统计学意义(P<0.01)。结论:行气利湿方联合金黄膏联合治疗可以明显改善湿热痹阻型膝骨关节炎患者的肿胀程度、疼痛程度及功能情况,并且能明显改善滑膜炎及积液的程度。
Abstract:
Objective:To investigate the effect of Xingqi Lishi prescription combined with Golden Paste on the treatment of damp-heat bi type knee osteoarthritis(KOA).Methods:A total of 105 patients with knee osteoarthritis who received treatment from January 2023 to June 2024 were randomly divided into combined treatment group(Xingqi Lishi prescription + Golden Paste),traditional Chinese medicine external use group(Golden Paste)and Western medicine group(oral Celeoxib capsule + Maizhili tablet),and the treatment was completed for 4 weeks.After 2 weeks of treatment and 4 weeks of treatment,the swelling degree(visual evaluation,floating patella test grade,circumference diameter of affected knee),pain degree(VAS score),functional assessment(HSS score),degree of synovitis(effusion thickness measurement,synovial thickness grade),WORMS score and so on were compared between the two groups.Results:The swelling degree,pain degree and functional scores of the three groups at 2 weeks and 4 weeks were better than before treatment(P<0.01),and the pain degree and functional scores of the combined treatment group were better than those of the external use group and the Western medicine group after treatment(P<0.01).The score of synovitis degree and WORMS in the combined treatment group were better than those in the external treatment group and the Western medicine group(P<0.01).Conclusion:The combination of Xingqi Lishi prescription and Golden Paste can effectively improve the swelling degree,pain degree and function of the patients with damp-heat obstruction KOA,and can significantly improve the degree of synovitis and effusion on imaging.
