摘要:
目的:分析InterTan髓内钉辅助钢板、钢丝行髓内外联合内固定治疗伴有冠状面骨折的A3.3型股骨粗隆间骨折的疗效。方法:回顾性分析2018年1月至2022年1月收治的56例伴有冠状面骨折的A3.3型股骨粗隆间骨折患者,采用InterTan髓内钉辅助钢板、钢丝髓内外联合内固定治疗。其中男22例,女34例; 年龄为62~92岁,平均为76.5岁; 对手术时间、术中出血量、骨折愈合时间等进行统计分析,末次随访采用Harris髋关节评分进行功能评价。结果:52例获随访,随访时间为12~24个月,平均为16.2个月; 所有患者骨折愈合良好,围手术期无严重并发症或合并症出现,骨折愈合时间为3~7个月,平均为3.8个月; 末次随访髋关节功能良好,Harris髋关节评分优42例(80.77%),良5例(9.62%),可4例(7.69%),差1例(1.92%),优良率为90.39%。结论:InterTan髓内钉辅助钢板、钢丝髓内外联合内固定方式能重建股骨外侧壁和控制旋转移位,固定可靠,疗效确切,并发症少,是治疗伴有冠状面骨折的A3.3型股骨粗隆间骨折的有效方法。
Abstract:
Objective:To observe the clinic effect of intramedullary and extramedullary internal fixation consist of InterTan associated with plant or steel wire internal fixation in the treatment of intertrochanteric fractures with coronal plane fracture.Methods:A retrospective analysis was performed on 56 patients with A3.3 type intertrochanteric fractures with coronal plane fracture were treated by InterTan associated with supplementary internal fixation of plant or steel wire from January 2018 to January 2022,including 22 males and 34 females aged from 6292 years old with an average of 76.5 years old.The operative time,intraoperative blood loss and fracture healing time were statistically analyzed.The function of hip joint were evaluated according to Harris hip scores at the last follow-up.Results:52 cases were followed up for 1224 months(mean 16.2 months).All patients had good fracture reduction,no coxal varus deformity,no serious complications in perioperative period,and the fracture healing time was 37 months(mean 3.8 months).According to Harris scoring standard,the hip function was excellent in 42 cases(80.77%),good in 5 cases(9.62%),fair in 4 cases(7.69%)and poor in 1 case(1.92%),with an excellent and good rate of 90.39%.Conclusion:InterTan intramedullary combined with auxiliary internal fixation can reconstruct the lateral wall of the femur and control the rotational displacement.The fixation is reliable and the postoperative complications are less.It is an effective method for the treatment of intertrochanteric fracture of the femur in the elderly with coronal plane fracture.
