摘要:
目的:研究兔髌骨全切除术后膝关节形态及病理改变。方法:20只新西兰大白兔随机分为空白组、对照组和观察组,其中观察组大白兔膝关节行髌骨全切除术。1年后处死动物,切取髌韧带、股骨滑车、股骨和胫骨关节面软骨和半月板,并观察形态学及组织学改变; 半月板切片苏木精-伊红染色,通过ImageJ图像处理分析软件定量分析半月板软骨陷窝深染的老化细胞面积。结果:空白组与对照组的股骨滑车沟深度和角度、Mankin评分、半月板软骨陷窝深染的老化细胞面积,差异无统计学意义(P>0.05)。观察组原髌骨所在的髌韧带无新生骨形成,股骨滑车软骨细胞病理表现相较于空白组和对照组退变明显,股骨滑车沟角度、Mankin评分及半月板软骨陷窝深染的老化细胞面积明显大于空白组和对照组,差异有统计学意义(P<0.05)。结论:髌骨全切除术后髌韧带无软骨再生,股骨滑车沟变浅,软骨及半月板退变加速,对后期可能进行的膝关节置换术影响深远,因此应慎重选择髌骨全切除术。
Abstract:
Objective:To investigate the morphology and pathological changes of the knee joint after total patellar resection in rabbits.Methods:20 New Zealand white rabbits were randomly divided into blank group,control group and observation group,and the observation group underwent total patellar resection of the knee joint.One year later,the animals were euthanized and the patellar ligament,femoral trochlear,femoral and tibial articular cartilage,and meniscus were harvested,and morphological and histological changes were observed.The hematoxylin eosin staining of meniscus slices was used to quantitatively analyze the aging cell area of deep stained meniscus cartilage pits using ImageJ image processing analysis software.Results:There was no statistically significant difference(P>0.05)in the depth and angle of the femoral trochlear groove,Mankin score,and the area of aged cells with deep staining of meniscus cartilage pits between the blank group and the control group.There was no new bone formation in the patellar ligament where the original patella was located in the observation group.The pathological manifestations of femoral trochlear chondrocytes showed significant degeneration compared to the blank group and control group.The angle of the femoral trochlear groove,Mankin score,and the area of deeply stained aging cells in the meniscus cartilage fossa were significantly larger than those in the blank group and the control group,and the difference was statistically significant(P<0.05).Conclusion:After total patellar resection,there is no cartilage regeneration in the patellar ligament,the trochlear groove of the femur becomes shallower,and the degeneration of cartilage and meniscus accelerates,which has a profound impact on the possible knee replacement surgery in the later stage.Therefore,total patellar resection should be carefully selected.
