摘要:
目的:探讨大直径自体腘绳肌腱重建前交叉韧带的有效性及安全性。方法:回顾性分析2022年6月至2023年6月诊治的34例采用大直径自体腘绳肌腱重建前交叉韧带的患者,男30例,女4例; 平均年龄为(24.32±6.41)岁。记录术中半腱肌、股薄肌长度及移植物的直径,术前、术后1年膝关节的IKDC评分和 Lysholm评分,Lachman试验及轴移试验情况。结果:半腱肌的平均有效长度为(26.04±1.33)cm,股薄肌腱的平均有效长度为(21.78±2.47)cm,6股移植物直径平均为(8.85±0.28)mm,8股移植物直径平均为(9.75±0.26)mm; 所有患者术后12个月IKDC评分和 Lysholm评分与术前相比,差异均有统计学意义(P<0.05); 6股和8股移植术后12个月IKDC评分和 Lysholm评分对比差异无统计学意义(P>0.05)。结论:采用大直径自体腘绳肌腱重建前交叉韧带能充分利用自体腘绳肌腱,确保移植物直径,胫骨侧采用门型钉及界面螺钉双固定的方式能确保固定牢固,近期疗效确切。
Abstract:
Objective:To explore the effectiveness and safety of large diameter autologous hamstring tendon reconstruction of anterior cruciate ligament.Methods:Retrospective analysis of 34 patients diagnosed and treated from June 2022 to June 2023,who underwent large diameter autologous hamstring tendon reconstruction of anterior cruciate ligament.There were 30 males and 4 females with an average age of(24.32±6.41)years old.Record the length of the semitendinosus and gracilis muscles during surgery,as well as the diameter of the graft,IKDC score,Lysholm score,Lachman test,and axial displacement test of knee joint before surgery and 1 year after surgery.Results:The average effective length of the semitendinosus muscle is(26.04±1.33)cm.The average effective length of the gastrocnemius tendon is(21.78±2.47)cm.The average diameter of the 6-strand graft is(8.85±0.28)mm,and the average diameter of the 8-strand graft is(9.75±0.26)mm.The IKDC score and Lysholm score of all patients at 12 months after surgery showed significant statistical differences compared to preoperative levels(P<0.05).There was no significant difference in the IKDC score and Lysholm score between 6 and 8 shares at 12 months after surgery(P>0.05).Conclusion:Using large-diameter autologous hamstring tendons to reconstruct the anterior cruciate ligament can fully utilize the autologous hamstring tendons and ensure the diameter of the graft.The tibial side is fixed firmly with a double fixation method of door nail and interface screw,which has a definite therapeutic effect.
