摘要:
目的:探讨经皮钳夹复位在股骨远端骨折术中应用的安全性和有效性。方法:回顾性分析2019年1月至2022年12月采用经皮钳夹复位结合逆行髓内钉治疗的36例股骨远端骨折患者资料。男20例,女16例; 年龄为25～88岁,平均年龄为63.5岁; 左侧17例,右侧19例。按AO/OTA分型:A2.1型10例,A2.2型12例,A3.1型9例,C1.1型5例。受伤原因:交通伤6例,摔伤23例,坠落伤7例。所有股骨远端骨折患者使用经皮钳夹复位,术中透视确认复位满意后再行闭合复位逆行髓内钉内固定治疗。术中观察血管神经损伤情况,术后观察骨折复位情况、骨折愈合时间和术后并发症。骨折愈合时进行膝关节功能Schatzker-Lambert评分。结果:36例均获得随访,随访时间为8～14个月,中位数为12个月。无血管神经损伤的病例,无感染病例,术后X 线片示骨折端对位对线良好。所有病例愈合良好,未出现骨折不愈合和延迟愈合病例。骨折愈合时间为12～24周,中位数为16周。Schatzker-Lambert功能评分:优25例,良8例,中3例,优良率为91.67%。结论:经皮钳夹复位应用于股骨远端骨折术中,具有操作简单、微创、安全、高效等优点,可缩短手术时间,减少术中透视次数和术中出血量,有助于后续手术操作的顺利开展,提高了临床疗效,值得临床推广应用。
Abstract:
Objective:To investigate the safety and efficacy of percutaneous clamp reduction in the operation of distal femoral fracture.Methods:The data of 36 patients with distal femoral fracture treated by percutaneous clamp reduction combined with retrograde intramedullary nail from January 2019 to December 2022 were retrospectively analyzed.There were 20 males and 16 females.Age ranged from 25 to 88 years old,with an average age of 63.5 years old.There were 17 cases on the left side and 19 on the right side.According to AO/OTA classification,A2.1 type was 10 cases,A2.2 type 12 cases,A3.1 type 9 cases,C1.1 type 5 cases.The causes of injury were traffic injury in 6 cases,fall injury in 23 cases and drop injury in 7 cases.All patients with distal femoral fractures were treated with percutaneous clamp reduction,and then with closed reduction and retrograde intramedullary nail fixation after the reduction was satisfactory.The injury of blood vessels and nerves was observed during operation.The reduction of fracture,healing time of fracture and postoperative complications were observed after operation.Schatzker-Lambert score of knee function was performed at fracture healing.Results:All 36 cases were followed up for 8 to 14 months,with a median of 12 months.There were no cases of vascular and nerve injury.No cases of infection; Postoperative radiographs showed good alignment of fracture end.All cases healed well,and no cases of non-union or delayed union occurred.The healing time of fractures was 1224 weeks,with a median of 16 weeks.Schatzker-Lambert function score:excellent 25 cases,good 8 cases,medium 3 cases,excellent and good rate of 91.67%.Conclusion:The application of percutaneous clamp reduction in the operation of distal femoral fracture has the advantages of simple operation,minimally invasive,safe and efficient,shortening operation time,reducing intraoperative fluoroscopy times and blood loss,contributing to the smooth development of subsequent operations,improving clinical efficacy,and worthy of clinical application.
