摘要:
目的:评价手法整复硬纸夹板外固定治疗Salter-HarrisⅠ型和Ⅱ型桡骨远端骨骺骨折的临床疗效。方法:选择2017年9月至2022年9月收治的桡骨远端骨骺骨折患儿26例,采取手法复位硬纸夹板外固定治疗。治疗后3个月、6个月、12个月共3次随访,根据X线片评估骨折复位和愈合情况,采用Gartland-Welley评分标准评价腕关节功能。结果:26例儿童桡骨远端骨骺骨折患者均达到骨性临床愈合标准,下尺桡关节对位关系正常,两侧桡骨等长,尺偏角、掌倾角正常。根据Gartland-Welley评分结果:优21例,良5例,优良率为100%。结论:儿童桡骨远端骨骺骨折大部分可以通过手法整复硬纸夹板外固定的治疗方法,取得满意疗效,硬纸夹板外固定法在儿童桡骨远端骨骺骨折临床治疗中具有疗效确切、安全性高、并发症少的特点,值得临床推广应用。
Abstract:
Objective:To evaluate the clinical efficacy of manual reduction and external fixation with cardboard splint on the treatment of Salter-Harris type Ⅰ and type Ⅱ distal radius epiphyseal fractures.Methods:26 children patients with distal radius epiphyseal fractures from September 2017 to September 2022 were selected.All patients were treated with manual reduction and external fixation with cardboard splint.The patients were followed up 3 months,6 months and 12 months after treatment.Fracture reduction and healing were evaluated according to X-ray films,and wrist function was evaluated according to Gartland-Welley scoring criteria.Results:All 26 children with distal radius epiphyseal fractures reached the clinical bone union standard,with normal distal radioulnar joint relationship,equal radius on both sides,normal ulnar inclination angle and palmar inclination angle.According to Gartland-Welley score,21 cases were excellent and 5 cases were good,with an excellent and good rate of 100%.Conclusion:Most of the distal radius epiphyseal fractures in children can be treated by manual reduction and cardboard splint external fixation.The method can achieve satisfactory clinical efficacy.The special cardboard splint external fixation on the clinical treatment of distal radius epiphyseal fractures in children has exact curative efficacy,high safety,and few complications,which is worthy of clinical promotion.
