摘要:
目的:探讨斜外侧腰椎椎间融合术(OLIF)治疗退行性腰椎管狭窄的近期效果及对患者血清TNF-α,IL-6,CRP水平的影响。方法:选择本院2017年7月至2018年6月间收治的60例退行性腰椎管狭窄患者临床资料为对象,分析OLIF对退行性腰椎管狭窄的治疗效果及术后3个月、6个月VAS评分、ODI指数、SF-36评分,血清TNF-α,IL-6,CRP水平变化情况。结果:所有患者均获得6个月随访,CT评估结果显示患者左、右两侧椎间孔间隙高度较术前均显著增高,左、右两侧椎间孔面积较术前均显著增大,差异有统计学意义(P<0.001); MRI评估结果显示患者椎管内径大小、盘黄间隙高度较术前均显著升高,椎管面积较术前显著增大,差异有统计学意义(P<0.001); 术后3个月及6个月末次随访,患者腰痛VAS评分、腿痛VAS评分、ODI指数,血清TNF-α,IL-6,CRP水平相较于术前均显著降低,差异有统计学意义(P<0.001),术后3个月及6个月末次随访SF-36评分相较于术前均显著升高,差异有统计学意义(P<0.001); 术后仅出现下肢麻木2例(3.33%),屈髋乏力1例(1.67%); 术后1个月出现融合器向下位移导致的腰痛1例(1.67%)。结论:斜外侧腰椎椎间融合术可显著改善退行性腰椎管狭窄患者椎管内空间结构及术后腰腿疼痛程度,减轻腰椎功能障碍,提高生活质量,且术后并发症少,近期疗效满意。
