摘要:
目的:探究冲击波联合激痛点臭氧注射对颈肩肌筋膜疼痛综合征(MPS)的临床疗效。方法:选取2016年5月至2018年5月接受治疗的颈肩MPS患者共60例,通过随机数字表法分为观察组和对照组(每组各30例)。对照组患者接受发散式体外冲击波单独治疗,观察组在对照组的基础上联合激痛点臭氧注射方法治疗,比较两组患者治疗总有效率、治疗前后的疼痛视觉模拟评分(VAS)、颈椎活动度(ROM)评分以及疾病复发率。结果:观察组显效+有效例数28例(总有效率93.33%),明显高于对照组22例(总有效率73.33%),差异有统计学意义(P<0.05); 治疗前,两组患者VAS评分比较差异无统计学意义(P>0.05); 治疗后7,14,21 d的VAS评分中,观察组均明显低于对照组,差异有统计学意义(P<0.05),两组患者VAS评分均呈现逐渐下降趋势; 两组患者治疗前,治疗后7,14,21 d的ROM评分比较差异无统计学意义; 观察组疗程结束后6个月内复发病例1例(复发率3.33%),对照组复发病例6例(复发率20.00%),差异有统计学意义(P<0.05)。结论:传统冲击波治疗颈肩MPS存在一定局限性,冲击波联合激痛点臭氧注射方法可进一步缓解患者疼痛,且根据臭氧本身化学性质,可从根本上修复患者组织损伤,复发率低,安全有效。
Abstract:
Objective:To investigate the clinical effect of shock wave combined with MTRP ozone injection in treating cervical and shoulder myofascial pain syndrome.Methods:60 patients with neck and shoulder myofascial pain syndrome in our hospital were selected from May 2016 to May 2018 and divided into observation group(n=30)and control group(n=30)by random number table method.Patients in the control group accepted separate treatment with divergent extracorporeal shock wave,and the observation group accepted combined treatment with MTRF ozone injection on the basis of treatment of the control group.The total therapeutic efficiency,visual analogue scale(VAS),cervical range of motion(ROM)score before and after treatment and disease recurrence rate of the two groups were documented and compared.Results:The total effective rate of the observation group was 93.33% which was significantly higher than that of the control group of 73.33%(P<0.05).There was no significant difference in VAS scores between the two groups before treatment(P>0.05).VAS scores at 7,14 and 21 d after treatment were significantly lower in the observation group than that of the control group(P<0.05).No significant difference in the ROM scores of the two groups was found at 7,14 and 21 d(P>0.05).In the observation group,there was a total of 1 recurrence case within 6 months after the end of treatment period with a recurrence rate of 3.33%,while there were 6 recurrence cases in the control group with a recurrence rate of 20.00%.Therefore,this difference was statistically significant(P<0.05).Conclusion:Traditional shockwave treatment of MPS in neck and shoulder has some limitations.Shock wave combined with ozone injection at pain point can further relieve the pain of patients,which can fundamentally repair the tissue damage of patients with a low recurrence rate and safety.
