摘要:
目的:观察桡、尺骨远端骨巨细胞瘤不同手术治疗方式的效果。方法:近5年本科成功随访8例桡、尺骨远端骨巨细胞瘤患者(桡骨远端5例、尺骨远端3例,其中初发4例、复发4例),3例尺骨远端病变患者进行了瘤段切除术,3例桡骨远端病变患者采用了瘤段切除+自体腓骨移植术,1例采用了瘤段切除+同种异体骨植入+腕关节融合术,另1例接受了截肢术。术后随访患者关节功能及复发情况。结果:患者术后均获得随访,时间12～60个月。7例行保肢术的患者在随访期间均未见复发。MSTS评分:优4例,良2例,中1例。术后腕关节稳定性、活动度均良好,能够胜任日常生活。2例患者在接受自体腓骨移植后出现骨不连,行自体髂骨植骨术治疗后骨愈合。结论:对桡、尺骨远端骨巨细胞瘤的治疗应根据患者临床、影像学、病理学等特点全方位考虑,选择合理的治疗方法,在肿瘤组织彻底清除的情况下尽量保全腕关节功能。其中,瘤段切除术后自体腓骨移植是重建腕关节的安全、有效的术式。
