小切口防旋股骨近端髓内钉治疗A3型股骨转子间骨折

摘要:
目的:评价采用小切口防旋股骨近端髓内钉(Proximal Femoral Nail Anti-rotation,PFNA)治疗A3型股骨转子间骨折的临床疗效。方法:2009年6月至2012年12月136例A3型股骨转子间骨折,按AO分型:A3.1型36例; A3.2型24例; A3.3型76例。分别采用闭合复位、小切口辅助复位+防旋股骨近端髓内钉治疗。分别统计手术时间、术中输血量、透视次数、并发症及术后疗效。结果:1)手术时间、术后并发症:组间比较差异无统计学意义(P>0.05); 2)术中出血量、术中透视次数:组间比较均有显著差异(P<0.05),术中出血量小切口组>闭合复位组,术中透视次数小切口组<闭合复位组; 差异有统计学意义(P<0.01); 3)疗效比较:闭合复位组及小切口组优良率分别为90.4%和93.7%,组间差异无统计学意义(P>0.05)。结论:两种复位方法结合PFNA治疗A3股骨转子间骨折,均可取得良好疗效。小切口感触辅助复位,有利于术中纠正髋内翻、近端旋转,大大减少术中透视,同时还可用作骨折端植骨的通道,植入自体股骨转子部开口扩髓时产生的大量优质松质,一举两得。通过经验和随访病例证实了小切口辅助复位治疗A3型股骨转子间骨折的优势性,值得临床应用。
